You may be discharged due to a number of reasons:
- You have successfully completed the programme
- We have not been able to contact you over a period of time via multiple different communication channels including phone calls, SMS and emails
- You are not engaging with your programme
- You are not eligible for the programme
- We have not received key information from your GP to enrol you on the programme
If you have been discharged you and your GP will receive an email confirming this and the reason why. We advise you speak with your GP about other support you may need, and this could include a re-referral to our service when appropriate.
