Why have I been discharged from the programme?

If you have started the programme, you may be discharged for one of the following reasons:

- You have successfully completed the programme.
- You are not engaging with the programme.
- We have been unable to contact you despite repeated attempts via phone, SMS, and email.

If you have not started the programme, you may be discharged for one of the following reasons:

- You are not eligible for the programme
- We have not received the necessary information from your GP to enrol you.
- We have been unable to contact you via multiple channels including phone, SMS, and email.

If you have been discharged, both you and your GP will receive an email confirming this. We recommend discussing next steps with your GP, who can advise on alternative support options. If appropriate, this may include a re-referral to our service.
